Awais Siddique, MD

Taha Siders, MD
Director of OperationsDr. Siders grew up in Dubai. He is married has two young children. He worked in the medical device industry for 7 years, and recently moved from the East Coast to start working as the Director of Operations for AZH. Dr. Siders lived in Cambridge, England for a few years to pursue his studies in Medical Education. He enjoys playing tennis and loves to travel.

LORI L. BROOKS
MDDr. Brooks is a Physical Medicine and Rehabilitation board-certified physician. She cares for patients of various ages who have chronic diseases that impede their functioning abilities.
Dr. Brooks' areas of expertise include wound treatment, amputees, wheel chair management, bracing, and neurological diseases.
Dr. Brooks' approach as a physiatrist is to concentrate on the whole person and examine how function affects her patient's well being.

Terri, RN
Registered NurseTerri is the Lead Wound Care and Hyperbaric Nurse for AZH Centers and has held this position since 2016. She received her Bachelor of Science in Nursing from Cardinal Stritch University in 1996, and Master of Science in Nursing from Marquette University in 2014, with a focus on leadership and quality improvement. She is a Certified Wound Specialist (CWS) in 2016, with over 16 years of advanced wound care and hyperbaric experience. Terri provides AZH patients with knowledge, experience and compassion not found elsewhere. She is quoted as saying “I enjoy the science of wound care because it’s amazing to watch the body repair itself, and fortunate enough to use advanced products and treatments at AZH.” Terri’s husband is proudly serving the US as a distinguished member of our Armed Forces.

Robyn
Medical AssistantRobyn is a Registered Medical Assistant and has been employed at AZH since June, 2016. Robyn is a 1998 graduate of Waukesha County Technical College. She brings 20 years of patient care experience with a background in urology, urogynecology and fertility. She has really enjoyed her work experience so far at AZH and is looking forward to becoming a hyperbaric technician and advancing her skills in wound care and vascular. In her free time she enjoys spending time with her husband, 2 children and yellow lab Murphy.

Deena
Lead HBO TechnicianDeena has been a certified Hyperbaric Technician since 2005. She has over 20 years of experience with hyperbaric medicine, as well as advanced wound care. Prior to hyperbaric and wound care, she spent over 15 years in the cardiac field. Deena has worked with Dr. Niezgoda for over 20 years. Deena says, “I enjoy the challenges and creativity wound care and hyperbaric presents, as well as the various dynamics our patients bring to us. My past time is spent with my grandchildren, singing and playing bells in our church choir, and traveling.”

Michelle
Front Desk/Medical AssistantMichelle has worked at AZH since we first opened, and has watched the company grow into the success it is today. She enjoys building long-lasting relationships with our patients. Michelle has been engaged in healthcare since she was 18, some of the facilities she has worked at are hospitals, assisted livings, home care, and more recently, an adult day program for adults with developmental disabilities.

Courtney
RN Vascular NurseCourtney is a Vascular RN, and has been with AZH Centers since 2018. Graduating from Alverno College in 2012 with her BSN in Nursing and support in psychology, she brings with her over 6 years of compassionate nursing experience caring for surgical patients. She states, “When a patient needs surgery, it can be an overwhelming and anxiety-ridden experience. It’s important for me to make my patients as comfortable and informed as possible through the whole process. It brings me joy to see patients improve their quality of life after a procedure.” In her limited spare time, she enjoys crafting, video games, and spending time with her friends and family.

Alya
Office AdministratorAlya started working at AZH in December of 2018. She is currently studying Communications at Arizona State’s online program. She plans to continue her future at AZH after graduation, focusing on the business aspect. In her free time, she enjoys to ride her horses, spend time with her friends and family, photography, and spend time outside.

Michaella Cariello-Becker
APNP, FNP-BC, CWSMichaella is a board certified Family Nurse Practitioner and Certified Wound Specialist (CWS) through the American Board of Wound Management, who joined AZH Centers in April 2019. She received her Bachelor’s of Science in Nursing from UW-Eau Claire and 2009, and her Master’s of Science in Nursing from Alverno College in 2017. Her background is in medical oncology and family practice. Michaella believes that AZH Centers is an “incredibly innovative clinic that consists of a multi-disciplinary team whose primary focus is the patient and their needs.” She also states, “I am a strong supporter of patients taking an active role in their health. I want to be able to provide patients with all the information they need to to make an informed decision and together we can make a plan of care.”
In her spare time Michaella loves spending time with her close family and friends, including her husband and two children. She also enjoys reading, trying new restaurants, and traveling.

Kathleen
RN Registered NurseKathleen is a Registered Nurse and has been with AZH since March of 2019. Kathleen graduated in 2009 from Gateway Technical College. She has 10 years of nursing experience with a background in neurology, hospice and home care nursing. Kathleen states, “During my experience working as a home care RN, I had the opportunity to care for patients that were also patients of AZH Wound and Vascular Centers. I was very impressed with the service the patients received and the exceptional healing of my patients wounds. As soon as I found out that AZH had an opening for an RN, I jumped at the chance to apply, and here I am. I am grateful to be working with such a wonderful group of exceptionally talented individuals!” In her free time, Kathleen enjoys spending her time with her husband Dennis, and her children.

Heather
Ultrasound TechnicianHeather is a Registered Diagnostic Medical Sonographer who has been employed at AZH since March 2019. She is a 2000 graduate of Concordia University Wisconsin and also graduated from the St. Francis Hospital School of Diagnostic Medical Sonography in 2002. Heather thoroughly enjoys getting to know all the very friendly patients at AZH.

Jessica
Medical AssistantJessica is a trained Medical Assistant and has been employed at AZH since May, 2019. Jessica has 10 years of patient care experience, with a background in long term acute care in hospitals, orthopedics, and pain management. In July of 2019, she received a Phlebotomy Certificate from Moraine Park Technical College. She has really enjoyed working with the wonderful staff at AZH and is excited to be apart of the team. In her free time, she enjoys spending time with family and friends.

William
Medical AssistantWilliam is a trained Medical Assistant who joined AZH in July 2020. He is a recent graduate from the University of Wisconsin – Madison and plans to continue his education next fall as a student of medicine. He states, “AZH recognizes the importance of a holistic approach. Our coordinated care team not only focuses on the wounds and ailments the patient may present but inquiries into possible environmental, lifestyle, nutritional, or vascular compromises which may have contributed to the problem they seek to remedy. AZH is medicine in its purest, most complete form.” A fun fact about William is that he is a sports enthusiast and the proud commissioner of his family Fantasy Football League.

Patient Care CoordinatorVeronica is new to the Milwaukee area and started with AZH in 2021. She has been in the medical field since 2018 working as a medical administrative assistant. Veronica enjoys spending time with family and enjoys road trips.
